Driver Licence Reports

First-time Learner Permit applications must have an accompanying eyesight report form D502.

This form can be downloaded by clicking here.

This form must be signed in the presence of the Optometrist at Gildeas Opticians.

As well as having good central vision it is important that drivers have effective peripheral vision and contrast sensitivity which is important for driving in poor light.

We can also carry out the more extensive test required for Group 2 (Trucks/buses etc) licences.

Licence holders, who are renewing their licence but have had a change in vision since the previous licence, also need to have the D502 form completed.

It is important to note that if you reach the visual stands only with an optical correction, it is a legal requirement to use it.
